The cost of the national healthcare system is more or less in line with the OECD average, while the quality level is above average. There are therefore not very high levels of waste, but in perspective there is a "very serious problem of financial sustainability" to which is added a growing problem of access to services. This was stated by the director general of ANIA, Dario Focarelli, in a hearing before the Budget and Social Affairs commissions of the Chamber. In this hearing Focarelli illustrated the results of “the fact-finding survey on the challenge of health protection between new needs of the health system and public finance objectives” created, in fact, by Ania.

Focarelli added that "an integrative system is absolutely necessary to guarantee the equity and universality of access to public health". According to the OECD, Ania recalled, in 2010 10-12% of people with the lowest income reported lack of access, which rises to 20% for dental care (the highest levels in comparison with large countries).

As for costs, Focarelli underlined, "all forecasts indicate a very strong increase in funding for public health". According to the State General Accounting Office, in 2060 there will be an increase of at least 2 or 3 points of GDP and 1,5-2 points for non-self-sufficiency.
